Denture Marking Requirement
Placement of identifying marks on a dental prosthesis. Responsibilities of dentist. Each dentist licensed in this state, who either makes or directs to be made a removable prosthetic denture, bridge, appliance or other structure to be worn in a person's mouth, shall offer to the patient for whom the prosthesis is to be made the opportunity to have such prosthesis marked with the patient's name or initials. Such markings shall be accomplished at the time the prosthesis is made and the location and methods used to apply or implant such markings shall be determined by the dentist or person directed to act on behalf of the dentist. Such marking shall be permanent, legible and cosmetically acceptable. A dentist shall advise the patient of any additional charges that may be incurred to obtain such markings on the prosthesis. Notwithstanding the provisions of this section, if in the professional judgment of the dentist or the entity that is making the prosthesis, such markings are not practicable or clinically safe, the identifying marks may be omitted entirely.

Denture Marking Requirement
(2) Every licensed dentist in this State making or directing to be made a removable prosthetic device, bridge, appliance or other structure to be used and worn as a substitute for natural teeth and/or supporting structure shall offer to the patient for whom the prosthesis is intended the opportunity to have such prosthesis marked at the time of fabrication. The location and method used for marking the prosthesis shall be determined by the dentist and this marking shall be permanent, legible, and cosmetically acceptable and shall include the patient’s name.
(3) If the dentist determines that identification is not practicable or clinically safe, the offer to mark the prosthesis need not be made.
(4) Any removable dental prosthesis fabricated prior to the effective date of this rule, shall be subject to the provisions of subsection (2) during a laboratory relining or rebasing of the prosthesis.

Denture Marking Requirement

(a) Every complete upper and lower denture and removable permanent partial denture fabricated by a dentist licensed in Georgia shall be marked with the name or social security number of the patient for whom it is intended. The marking shall be done during fabrication and shall be permanent, legible, and cosmetically acceptable. The exact location of the marking and the methods used to apply or implant it shall be determined by the dentist.

(b) If, in the professional judgment of the dentist, this marking is not practicable, the marking shall be as follows:

(1) The initials of the patient may be shown if the use of the full name or social security number is impossible; or

(2) The marking may be omitted entirely if none of the markings so specified are practical or clinically safe.

(c) Any removable dental prosthesis in existence prior to July 1, 1988, shall be marked in accordance with this Code section at the time of any subsequent rebasing.

(d) It shall be the duty of the Georgia Board of Dentistry to notify each person licensed to practice dentistry in this state of the requirements of this Code section. Such notification shall be mailed to the address of record of each person licensed to practice dentistry in this state.

Denture Marking Requirement
(a) General rule.--Every complete upper and lower denture, removable partial dental prosthesis and removable acrylic orthodontic appliance fabricated by a dentist licensed in this Commonwealth or fabricated pursuant to the dentist's work order or under the dentist's direction or supervision shall be marked with the name of the patient for whom the denture is intended. The markings shall be done during the fabrication and shall be permanent, legible and cosmetically acceptable. The dentist or the dental laboratory fabricating the upper and lower denture, removable partial dental prosthesis or removable acrylic orthodontic appliance shall determine the exact location of the markings and the methods used to apply or implant them.
(b) Exceptions.--
(1) If in the professional judgment of the dentist or the dental laboratory this identification is not practical
because the name cannot be placed in an obscure area, identification may be shown with the initials of the patient.
(2) If in the professional judgment of the dentist or the dental laboratory during manufacture or repair of the
denture it is determined that the forms of identification identified above are not safe for the patient, the
identification requirement established by subsection (a) is waived.
(3) Full or removable dentures that are relined are not subject to the identification requirement established by
subsection (a).
Section 3. Subsequent marking. Any removable upper and lower denture, removable partial dental prosthesis, or removable acrylic orthodontic appliance in existence before the effective date of this section that was not
marked in accordance with section 1 at the time of its fabrication shall be marked at the time of any subsequent
rebasing.
Section 4. Patient's exemption. If the patient or legal guardian for the patient shall for reasons of privacy or personal wishes refuse to have any upper and lower denture, removable partial dental prosthesis, or removable acrylic orthodontic appliance covered under this act to be labeled, a signed and dated copy of a release to the effect shall accompany the denture to the laboratory fabricating the denture. The original signed and dated release shall be kept
in the patient's dental record.

Denture Marking Requirement
§116.10. Prosthetic Identification. (a) It shall be the duty of the licensed dentist to insure that all removable prosthetic devices or removable orthodontic appliances delivered to a patient under his or her care bear a permanent identification marking suitable to determine that the prosthetic device or removable orthodontic appliance belongs to that patient.
(b) A suitable marking shall be defined as one that includes the patient's last name and first initial, and/or social security number. This marking shall be placed in the denture base of the removable prosthetic device or acrylic portion of the removable orthodontic appliance in such a manner as not to compromise the aesthetics of the restoration. (c) The licensed dentist shall install this identification marking or shall request on the prescription to a registered dental laboratory that the laboratory place the identification marking in the removable prosthetic device or removable orthodontic appliance. (d) Nothing in this rule shall preclude a dental laboratory from charging a fee for this service. (e) This rule shall not apply to any removable prosthetic device or removable orthodontic appliance that contains no acrylic, vinyl or plastic denture base, or if said appliance is too small to reasonably accomplish this procedure. (f) A dental laboratory that is required to register with the State Board of Dental Examiners shall clearly label or certify in writing to the prescribing dentist that the prosthesis or appliance being delivered to the prescribing dentist was either: (1) manufactured entirely by the SBDE registered dental laboratory; (2) manufactured in part or whole by a domestic laboratory inside of the United States; or, (3) manufactured in part or whole by a foreign laboratory outside of the United States and clearly identify the country in which the dental laboratory work was performed. (g) A dental laboratory that is required to register with the State Board of Dental Examiners may return to the dentist who issued the prescription written certification of all materials utilized in the prosthesis or appliance, including the percentage of each ingredient used in the fabrication of the prosthesis or appliance.

Denture Marking Requirement
Every complete upper and lower denture and removable dental prosthesis fabricated by a dentist licensed under this chapter, or fabricated pursuant to the dentist's work order or under the dentist's direction or supervision, shall be marked with the name of the patient for whom the prosthesis is intended. The markings shall be done during fabrication and shall be permanent, legible, and cosmetically acceptable. The exact location of the markings and the methods used to apply or implant them shall be determined by the dentist or dental laboratory fabricating the prosthesis. If in the professional judgment of the dentist or dental laboratory, this identification is not practical, identification shall be provided as follows:
(1) The initials of the patient may be shown alone if use of the name of the patient is impracticable; or
(2) The identification marks may be omitted in their entirety if none of the forms of identification specified in subsection (1) of this section is practicable or clinically safe.

Denture Marking Requirement
(1) Except as provided in sub. (2), a dentist who constructs a removable prosthetic device shall mark the device with the patient's first and last name. Except as provided in sub. (2), a dentist who authorizes a dental laboratory or dental laboratory technician to construct a removable prosthetic device shall ensure that the device is marked with the patient's first and last name. (2) The following exceptions apply to the identification required under sub. (1): (a) The first, middle, and last name initials of the patient may be substituted for the first and last name of the patient if, in the professional judgment of the dentist, it is impracticable to mark the first and last name of the patient. (b) The name and the initials of the patient may be omitted if each of those forms of identification is medically contraindicated.
