Term of Agreement: This Agreement becomes effective on the date you sign and remains in effect for three (3) years from that date.

By signing below, I acknowledge that during my visit to Global Dental Science LLC (dba AvaDent Digital Dentures), I may have access to confidential or proprietary information, including designs, technology, and patient-related materials. I agree not to disclose, reproduce, or use this information for any purpose other than my authorized visit.

I understand that this agreement will remain in effect for three (3) years from the date of my signature and is governed by the laws of the State of Arizona. I agree to return or destroy any confidential materials provided to me upon request or at the end of my visit.

This agreement represents the entire understanding between the parties regarding confidentiality and supersedes any prior discussions or agreements on this subject.
